In the form (online)Name of the applicant should be mine, right ? 2. name of the person receiving certificate - need not to be filled, right ? 3. purpose - can I leave it blank or NA ??? 4. date of birth should be of Baby (of course but it is mentioned along with mother's details) right ? 5. point # 14 and 15 are confusing... if birth was in hospital, I don't have to fill place of birth -right ??? 6. what is date of registration and registration number - is it hospital number ???
1. Yes, you are the applicant 2. Yes, but if you have some other person coming to collect then put that name. Otherwise you can put your name. I had put my name. 3. Purpose you can add "school admission, passport". I am not sure if you can put blank. 4. Yes correct 5. Place of birth is the place of hospital. Just fill that. 6. No need to fill registration details etc. Hospital records are sent directly online...
Hi Kethan, very informative blog! Recently, i got my daughters birth certificate. In the certificate Point 5. above. Place of birth is mentioned as Hospital name only and not city. My doubt is whether it should be city name? Because in future, place of birth for example we write as "Bangalore, Karnataka" or some thing like this. whats your input to this?
When you write hospital name, you can include city name as well. For e.g. "Manipal Hospital, Bangalore". But it has no bearing on the certificate that you receive.
Note that the city/state name is already included in the "Birth Certificate" since you can only get the certificate from local authorities. Check out the sample certificate http://dadsmemoir.blogspot.in/2012/02/bangalore-birth-certificate.html
The top line mentions that this is from BBMP Bangalore Karnataka certificate.

Thanks for the details as there are not many in the net. My suggestion to, to be fathers would be to get the birth certificate within 30days of child's birth. The information about child's birth (birth record) reaches from the hospital to the nearest corporation office in 5-7days. Parent needs to go to the corporation office, give the child's name, pay the fees (Rs110/-) and get the certificate in just 15minutes.
Hi Ketan I got most of the replies I was looking for before I visit the BBMP Upparpet Police station. Writing this in the afternoon and was wondering whether the office operates post lunch? Office timing for the application acceptance/certificate collection would have been great! /Guru
Yes I would expect the office operates post lunch also except on saturdays. These should be usual government office timings. Sorry I have not visited post-lunch though. Hope that helps.

You will have to fill one form (you can get the form there itself), pay the amount (Rs.140) in the first floor counter. They will give some date when you can come to collect the birth certificate (I think usually 10 days. They asked me to come on on Aug 13th).
Yes, office works post-lunch too. They work till 5PM like any other state government office. Second Saturday is holiday.
My daughter was born in Manipal hospital in 2009 Dec. I have got her birth certificate from BBMP. But now some schools are asking for the hospitals birth certificate (ie. Manipal hospital) . I dont have one. What should i do? Thanks in advance. Lalitha
I am surprised that schools are asking for hospital birth certificate. You can ask them to give it in writing that birth certificate from BBMP is not acceptable and they want certificate from hospital.
You can try approaching the medical records section (ground floor) in Manipal Hospital if they can provide another copy.
I was feeling nervous to go BBMP/Govt. office till some days back. Now your post made me strong enough to go BBMP and apply for Birth Certificate :)
My daughter born 5 months back at Sagar Hospitals, Jayanagar. I waited till naming ceremony to apply for Birth Certificate including name. Today I went to Megestic BBMP office and the guy at megestic BBMP told that birth details of year 2012 not yet come to their office and advised to me to go Jayanagar BBMP.
Then went to Jayanagar BBMP and and applied for 5 certificates, The guy at counter took Rs.120 and told me they can give max 2 certificates that too after 15 days.
Here one thing I didn't get. Since most of the hospitals posting birth details online, why that guy asked me to go back to Jayanagar. They told same thing to one more person and asked to go back to other BBMP. To avoid more work..I guess..
hi kethan, this is ashok, i'm 24 years old,i was born in KC GENERAL(MALLESHWARAM), my birth date was not registered, i wamt my birth certificate to apply for birth certificate, i started the process to obtain my passport a month before, but i've not yet got it.
i'm bit afraid now that i can get my birth certificate or not.
so please tell me the process to apply for birth certificate. ( remember my birth date was not registered).
I got my new born's certificate from the kasturi nagar office. As per Manipal I should have gone after 1 month from date of birth but after reading the info here I went after 20 days itself (less than a month from dob). The bbmp office was totally empty at 10 am and i got the cert in 7 minutes flat. The form we had filled at the hospital had no place to write the kid's name but i still wrote it on top of the form. In the bbmp form in Applicant's name the bbmp person told me to put my kid's name so as to include it on the cert straight away and not have to include the name later. Due to shortage of certificates, i got only 2 though i had asked for 5. I took the discharge summary along just in case asked.
My son was born in Cradle Jayanagar in on 29th June 2011 (its more than a year now). I have got a birth certificate from hospital without a name on it as we have not named him by then. But I have not taken the Birth certificate from BBMP office yet. Can you please guide me where should I go to get my son's birth certificate. Some of my friends said that I have to go to Mayo hall. Should I go to Mayo hall or should I go to BBMP office in Majestic. Also please help me in list of the documents I have to carry.
In the birth certificate I received, permanent address of parents is same as current address, should that be fine? as my permanent address is different, will it cause problems while applying for passport for baby?
Thanks for the info I am gonna go to BBMP office for my 2nd child but still nervous if they will give me in a fast way as with my first one they made my dad run couple of times for no reason and he had to wait for them in spite of paying money each time he visited them.

I had to pay 50/- and he asked me to come on 7th (after 3 days) @ 3pm. A stamped slip was handed over to me with the date and time.
Today, 7th Sep, I went to the BBMP office @ 3pm (Saturday). The person over there asked me to sign on a register and handed me 4 copies of the birth certificate. After that, he was busy attending to others. This is the fastest service that I received from any govt agency, and the least expensive (4 original copies = Rs.50/-). I was back home by 3.20pm.
Pl note that, I had gone to BBMP office within 1 month from the time, my son was born. I wish, all govt office operate the same way.

I was Blessed Baby girl In Bidar District, on 10-11-11 at Bidar, we have got birth certificates of my Daughter same from bidar Panchayath
but @ the time baby name was not decided, now it is two years completed, I want to add my daughter name, please help me..Iam at now In Bangalore, shall I do at Bangalore or I need to go my baby birth Place..??

It should not be a problem. When you apply for passport you need to give an address proof if the current address is different.
